No payment is required to book and no card is taken to hold a time. We call you the next business day to set up your sleep test; its $297 is paid on that call, before it ships, and cannot be refunded once it ships. The remaining $497 is paid at your visit. Please give us at least two business days’ notice to move your visit.
What you are booking is a diagnostic evaluation, not a prescription. Completing an assessment does not guarantee any particular treatment. All treatment decisions are made independently by the treating clinician based on your medical history, evaluation, diagnostic findings and clinical judgment. No outcome is promised or implied.
We are cash-pay and do not bill insurance. HSA and FSA receipts are provided. Services are for adults 18 and older located in Illinois.
