Always Tired, Men Over 40? 7 Causes Worth Testing
Most men over 40 who feel always tired, even after a full night in bed, have a cause that can be measured. The most common are sleep apnea, low testosterone, thyroid problems, anemia or low vitamin levels, blood sugar problems, medications and stress. Sleep apnea and low testosterone often show up together, so it pays to look at both.

Fatigue is not a character flaw, and it is rarely just age. It is a signal. The useful question is not whether you are tired, but why. For most men, the answer lives in two places: how you sleep and what is in your blood.
Why Are Men Over 40 Always Tired? The 7 Common Causes
Men over 40 are usually always tired for one of seven measurable reasons, and often for two or three at once. The Cleveland Clinic’s overview of fatigue names each of the causes below. The table shows the clues that tend to point to each one and how it is checked.
| Cause | Clues you might notice | How it is measured |
|---|---|---|
| Sleep apnea | Loud snoring, gasping at night, dozing off in meetings or at the wheel | A sleep study, in a lab or at home |
| Low testosterone | Lower energy and sex drive, irritability, poor concentration | Two early morning blood tests |
| Underactive thyroid | Tiredness with no clear trigger | Thyroid blood tests |
| Anemia, low vitamin D or B12 | Feeling drained by ordinary effort | Blood count and vitamin levels |
| Blood sugar problems | Fatigue alongside weight gain | Glucose and longer-term blood sugar markers |
| Medications and alcohol | Tiredness that started with a new prescription or habit | A structured review of what you take |
| Stress, burnout or low mood | Exhaustion with poor sleep, worry or loss of interest | A clinical conversation and screening |
No single symptom proves any one cause. That is the point of measuring instead of guessing.
Is It Fatigue or Sleepiness?
Fatigue and sleepiness are different, and the difference narrows the search. The Cleveland Clinic describes fatigue as extreme tiredness that makes it hard to get up and get through the day. Sleepiness is the urge to fall asleep when you should be awake.
If you nod off in meetings, on the couch by 8 PM or at red lights, that leans toward a sleep problem. The NIH’s National Library of Medicine review of obstructive sleep apnea lists excessive daytime sleepiness as one of its most common symptoms, alongside fatigue and morning headaches. If you feel drained but cannot nap, hormones, blood chemistry, medications and mood move higher on the list.
Why Am I Tired After 8 Hours of Sleep?
Because time in bed is not the same as restorative sleep. If your breathing is interrupted, or your hormones or blood chemistry are off, eight hours can still leave you running on empty.
Sleep apnea is the classic example. Breathing pauses can fragment the night without fully waking you, so you never remember them. The Cleveland Clinic also lists insomnia and shift work sleep disorder as sleep-related causes of fatigue. If the hours are there but the energy is not, sleep quality is the first thing worth checking.
Could Sleep Apnea Be Why I’m Always Tired?
It may be, and it is one of the most common and most missed causes. According to the same NIH review, 25% to 30% of men in the United States meet the criteria for obstructive sleep apnea. Its main risk factors include obesity, male sex, advancing age and a large neck circumference.
Signs worth taking seriously:
- Loud snoring, or a partner who notices you stop breathing
- Gasping or choking during the night
- Excessive daytime sleepiness, including drowsiness at the wheel
- Morning headaches
- Sleep that feels unrefreshing no matter how long it lasts
Clinicians often screen with the STOP-BANG questionnaire, which the NIH review calls one of the most widely accepted screening tools for sleep apnea. It asks about Snoring, Tiredness, Observed breathing pauses, blood Pressure, BMI, Age, Neck size and Gender. A screening score is a reason to test, not a diagnosis.
Weight matters here. Midsection weight gain, sleep and hormones tend to move together, which is why belly fat after 40 deserves a closer look in any man who is always tired.
Can Sleep Apnea Cause Low Testosterone?
The two are clearly linked, though cause and effect are still debated. A 2022 meta-analysis of 24 case-control studies found that men with obstructive sleep apnea had significantly lower testosterone than men without it. The authors also noted wide differences between studies, linked to age, BMI, race and testing method.
The Endocrine Society lists sleep disorders and obesity among the causes of low testosterone. It also reports that 30% to 50% of men with obesity or type 2 diabetes have hypogonadism, the medical term for low testosterone. Excess weight, poor sleep and low hormones can each feed the others.
This is the part most fatigue articles miss. A low testosterone result in a man with untested sleep apnea is only half the picture. The Endocrine Society notes that some causes of low testosterone, such as obesity, can be reversible without testosterone therapy and should be addressed before therapy is considered. In practice, that means testing sleep and hormones together and reading them side by side.
Can Low Testosterone Make You Tired?
Yes. The Endocrine Society lists reduced energy among the symptoms of low testosterone, along with lower sex drive, irritability, poor concentration and depressed mood.
It estimates that about 35% of men older than 45 have hypogonadism, and that testosterone levels naturally fall by about 1% each year after age 30. For a fuller symptom list, see 10 signs you may have low testosterone.
Diagnosis requires data, not guesswork. The Endocrine Society calls for at least two early morning blood tests, drawn between 7 and 10 AM, that show low testosterone, along with typical signs and symptoms. A single afternoon reading does not tell the full story.
What Other Health Issues Cause Constant Fatigue?
Several common conditions can drain energy in midlife men, and the Cleveland Clinic names each of these as a cause of fatigue:
- Thyroid. Hypothyroidism, an underactive thyroid, is among the hormonal causes.
- Anemia. Fewer healthy red blood cells means less oxygen delivered to muscle and brain.
- Vitamin D and B12. A deficiency in either is listed as a nutritional cause.
- Type 2 diabetes and obesity. Both appear on the list of chronic conditions linked to fatigue.
- Heart, lung and kidney disease. Less common, but they belong in the workup when fatigue comes with other symptoms.
Can Medications, Alcohol or Stress Cause Fatigue?
Yes. The Cleveland Clinic lists beta-blockers, sedatives, opioids, anticonvulsants, anti-anxiety medications and over-the-counter antihistamines among the drugs that can cause fatigue. It advises drinking alcohol in moderation, if at all. Never stop a prescribed medication on your own. Review it with the provider who prescribed it.
Stress, burnout, a sedentary routine and poor diet are also on its list, as are depression and anxiety. The trap is assuming stress explains everything and stopping there. Measuring the physical causes first makes the stress conversation clearer, not less important.
When Should a Man Over 40 See a Doctor About Fatigue?
See a clinician when fatigue lasts more than a few days, has no clear cause, came on suddenly, comes with unexplained weight loss, or makes daily life hard to manage. Those are the Cleveland Clinic’s own thresholds.
Seek immediate care if fatigue comes with shortness of breath, chest or arm pain, an abnormal or racing heartbeat, severe headache or vision changes, muscle weakness, or thoughts of harming yourself. If you are in crisis, call or text 988.
What Tests Check for Fatigue in Men?
Blood work and a sleep study together cover most of the common physical causes of fatigue in men. A sensible order of operations:
- Screen your sleep. Snoring, witnessed pauses or daytime dozing point toward a sleep study.
- Draw morning blood. Testosterone, thyroid markers, a blood count, blood sugar markers, vitamin D and B12 are common starting points.
- Confirm before concluding. A low testosterone result should be repeated in the morning before any diagnosis.
- Review what you take. Medications, alcohol and supplements, with a clinician.
- Read it together. Sleep, hormones, metabolism and body composition interact, so the pattern matters more than any one number.

On the sleep side, the American Academy of Sleep Medicine considers both in-lab polysomnography and home sleep apnea testing to be valid diagnostic options for uncomplicated adults at increased risk of moderate to severe sleep apnea. In-lab testing is recommended instead for men with significant heart or lung disease, chronic opioid use, a history of stroke, or severe insomnia.
How Alpha Refinery Approaches This
We treat fatigue as a data problem first. Our clinical team does not start with a treatment. It starts with a measurement.
In practice, that means one private 90-minute visit. We draw 61 blood biomarkers across hormone, metabolic, cardiovascular, inflammation and vitamin and nutrient categories, and run them in our own CLIA-certified lab. We add InBody 970 body composition, grip strength and blood pressure. When the history points to sleep, Alpha Sleep adds one night of sleep measured at home, so breathing and hormones are read side by side.
Results are read against your symptoms, not against a single cutoff. A borderline testosterone value is confirmed before it is acted on. Possible sleep apnea is evaluated before hormones are blamed. The output is a 90-day clinical protocol, and any treatment is evaluated by our clinical team and requires a prescription from a licensed provider. Our care is expert-led with physician medical oversight.
If you are a man in Hinsdale, Oak Brook or Naperville who is tired of guessing, start with the Alpha Score, a free two-minute self-check that shows where your energy, sleep and performance stand. When you are ready for data, the Alpha Performance Assessment is $497, with key available findings reviewed during your visit and nothing due to book. Take the Alpha Score, or Choose Your Time.
Common Questions
Is it normal to be tired all the time after 40?
Some change in energy with age is common, but constant fatigue is not something to accept by default. It often has a measurable cause, such as sleep apnea, low testosterone, thyroid or blood sugar problems, that is worth checking.
Can sleep apnea cause low testosterone?
Men with sleep apnea have significantly lower testosterone on average, according to a 2022 meta-analysis, though cause and effect are still debated. The Endocrine Society lists sleep disorders among the causes of low testosterone, which is why the two are best tested together.
What blood tests check for fatigue?
Common starting points include testosterone, thyroid markers, a blood count, blood sugar markers, vitamin D and B12. Which tests fit you is evaluated by our clinical team.
Is a home sleep test as accurate as a sleep lab?
In-lab polysomnography remains the gold standard. The American Academy of Sleep Medicine accepts home sleep apnea testing as a diagnostic option for uncomplicated adults at increased risk of moderate to severe sleep apnea, and recommends the lab for men with certain heart, lung or neurological conditions.
Is testosterone therapy the answer to fatigue?
Not automatically. Fatigue has many causes, and low testosterone is diagnosed with repeat morning blood tests plus symptoms. Any treatment requires an evaluation and a prescription from a licensed provider.
Key Takeaways
- Being always tired after 40 usually has a measurable cause, and often more than one.
- Sleepiness points toward a sleep problem; fatigue without sleepiness widens the search to hormones, blood and medications.
- About 25% to 30% of U.S. men meet the criteria for sleep apnea, and men with sleep apnea tend to have lower testosterone.
- Low testosterone is confirmed with two early morning blood tests plus symptoms, not a single reading.
- Testing sleep and blood together reveals overlapping causes that single tests can miss.
